    Mapleton Public Schools and Skyview Athletics have transitioned away from paper copy registrations and now all athletic registations, both high school and middle school, will now be completed online.   • Registration Requirements

    In order to participate in official practices and competitions at Mapleton, students must have clearance from the Skyview Athletics Department by filling out the Athletic Registration. All steps including payment will be online.

    Prior to registering any student-athlete, please make sure you have the following components in order to complete the registration process. Missing any of the components will limit your ability to complete the athletic registration.

    • An electronic copy of an active physical (a physical that was performed with the last 365 days)
      • This may include a PDF copy or a picture of the physical.
    • Information regarding medical insurance
      • Must include the insurance company name as well as the policy number
      • If you do not have medical insurance, please click here to be sent to our webpage regarding obtaining insurance through K&K Insurance.
    • A debit/credit card for payment


    The online registration has six parts. Anything that has an asterisk (*) next to it is a required field that must be completed in order to move to the next step. The following steps are:

    • Step 1 – Select Student
      • When filling out for the first time, you will need to enter the information regarding the student registering for athletics.
      • Once you have entered the information regarding the student, on any future return to the site to register will only require selecting the entered student from a list of students. 


    • Step 2 – Select Sport
      • This step is where you will select the sport the student-athlete what’s to competing in.
      • Sports will be active and selectable 3 weeks prior to the start of the season. 


    • Step 3 – Parent/Guardian Info
      • We require parent/guardian information for at least one parent/guardian.
      • A second parent/guardian information section is available but not required. 


    • Step 4 – Physical Forms
      • You must submit an electronic copy of an active physical.
        • Physicals are active for one (1) year from the date of the exam.
        • The electronic copy can be a PDF or an image. Please make sure the physical is readable and contains ALL parts. 


    • Step 5 – Medical Information
      • This section focuses on the medical information.
        • Most of this section is not required, however having information on allergies, medical conditions, and medication will greatly aid our athletic trainer in assisting the student-athlete.
      • Medical Insurance is a required field
        • You must include the name of the insurance company AND the policy number associated with your coverage.
      • You are required to provide at least one emergency contact. 


    • Step 6 – Parent Permission for Athletic Participation
      • This section includes the following disclaimers that need to be read and agreed to:
        • Parent Permission for Athletic Participation
        • Interscholastic Athletic Competition Code
        • Training Rules – Smoking, Use of Chewing Tobacco, or Possession of Drug Paraphernalia
        • Letters and Certificate (High School Only)
        • Athletic Fee
        • Skyview Athletics Concussion Waiver


    • Payment Step
      • After you have completed the registration, you will be sent to the payment section.
          • All payments will be completed online.
          • You must have an active debt or credit card.

Athletic Fees

  • Any student participating in athletics must pay an athletic fee. Payment is the last part of the registration and is required to finalize the registration process online.

    • High School Fee
      • $50 per sport, $100 maximum
    • Middle School Fee
      • $35 per sport, $70 maximum
