  • Registration Requirements

    To participate in official practices and competitions at Mapleton, students must have clearance from the Skyview Athletics Department. Students will need to submit the Athletic Registration packet in person to the Athletic Office at the Skyview Campus (8990 York St., Thornton 80229). The packet has three parts:

    • "Physician Permission/Parent Permission/Medical Insurance/Emergency Contacts" 
      • Must have a current physical examination form stamped and signed by a doctor licensed in the state of Colorado. Physicals are valid for one calendar year
      • "Parent Permission" section must be signed by parent/guardian and student 
      • Proof of medical insurance
        • Must include the insurance company and insurance policy number
      • Updated and accurate contact information
    • "Interscholastic Athletic Competition Code"
      • Must be signed at the top by parent/guardian and student
    • "Skyview Athletics Concussion Waiver"
      • The bottom of the form must be signed

    Additionally, any student participating in athletics must pay an athletic fee. Payments must be done with complete paperwork.

    • High School Fee
      • $40 per sport, $80 maximum
      • $35 per sport, $75 maximum (if a student receives free or reduced lunch)
    •  Middle School Fee
      • $20 per sport, $40 maximum
      • $15 per sport, $30 maximum (if a student receives free or reduced lunch)

    Payment can be cash or check- credit cards will not be accepted. Please make checks payable to: Skyview Athletics or Mapleton Public Schools. 

    ***Checks placed for collection will be subject to a $20 service charge***

    High School forms are available in the Athletic Office at the Skyview Campus (8990 York St., Thornton 80229). Middle School forms are available at your students school main office. All documents must be complete and on file in the Athletic Office before a student may participate.
