Health Services Asthma
If your child has Asthma, you will be given an Asthma Health Care Plan by your school nurse. Please sign and have it filled out and signed by your child’s doctor. Please check the Inhaler with the Health Assistant or front office. A Self-Carry contract must be completed with the Asthma Health Care Plan if you, the nurse, and your child’s doctor agree that your child can carry the inhaler. If your child has Asthma and you DO NOT want to have an inhaler at school, please contact the school district nurse consultant.Guidelines
