      The State of Colorado requires immunization records in order to enroll in school, Immunization Law (C.R.S. 25-4-902). Please check below to see if your child is up to date on immunizations and make an appointment with your doctor if needed. You will need to provide immunization paperwork/records to the school’s secretary or health assistant as required by the Colorado Department of Public Health and Environment; read their notice of Immunization requirements for school and childcare here.  

      Schools do have the authority to suspend or expel any student who is non-compliant under C.R.S 25-4-907. Please contact the Children's Hospital District Nurse Consultant if you have any questions or concerns.

      Colorado Immunization Information for Parents and Students

      Colorado law requires all students attending Colorado schools and licensed child care providers to be vaccinated against certain diseases unless an exemption is filed. For more information about vaccine requirements, resources and exemptions, visit the Colorado Department of Public Health and Environment’s School Immunization page.

      For additional information regarding immunizations Immunization Information for Parents

      Parents have the right to exempt their child from school immunization requirements according to C.R.S. 25-4-903. Parents need to submit to the school a statement of personal or religious exemption annually. A medical exemption must be signed by a licensed health care provider and only needs to be submitted once. For the latest update on non-medical exemptions please read this letter from CDPHE from April 2017 for clarification on exemptions: Immunization exemptions update from CDPHE (DOC). Non-medical exemptions must be submitted annually. Non-medical exemtions expire June 30th of each year. Medical and non-medical exemption forms can be found by clicking: www.colorado.gov/vaccineexemption. 

      COVID-19 Immunization Information 

      The Colorado Chapter of AAP (American Academy of Pediatrics) encourages health care providers to continue to provide care, especially well child visits and routine vaccination services. Reach out to your health care provider to discuss their health and safety protocols during COVID-19. The CDC (Centers for Dieasease Control) also continues to reccomend well-child care including immuniztions Vaccine Recommendations During the COVID-19 Pandemic.

      Colorado Immunization Law

      Colorado State Law requires students to provide an immunization record in order to be able to register for school.  At registration, all students will be expected to have an immunization record.
