CHSAA Physicals
According to CHSAA bylaw 1780.1, an adequate physical examination by a licensed practitioner is required within 365 days of sports participation. Starting in the 2025-2026 school year, the CHSAA Preparticipation Physical Examination (PPE) form with be the only accepted physical form for student-athletics.
All High School students planning on participating in athletics are required to use the CHSAA PPE form when registering. Going forward, any registration that uploads any other type of physical will not be approved until a CHSAA PPE form is submitted.
When uploading the CHSAA PPE form, you only need to upload Page 4.
