Immunizations
The State of Colorado requires immunization records in order to enroll in school, Immunization Law (C.R.S. 25-4-902). Please check below to see if your child is up to date on immunizations and make an appointment with your doctor if needed. You will need to provide immunization paperwork/records to the school’s secretary or health assistant as required by the Colorado Department of Public Health and Environment; read their notice of Immunization requirements for school and childcare here.
Schools do have the authority to suspend or expel any student who is non-compliant under C.R.S 25-4-907. Please contact Mapleton's Lead District Nurse, 303.853.1247, if you have any questions or concerns.
Colorado Immunization Information for Parents and Students
Colorado law requires all students attending Colorado schools and licensed childcare providers to be vaccinated against certain diseases unless an exemption is filed. Parents have the right to exempt their children from school immunization requirements according to C.R.S. 25-4-903. Parents need to submit to the school a statement of personal or religious exemption annually. A medical exemption must be signed by a licensed healthcare provider and only needs to be submitted once. For the latest update on non-medical exemptions please read this letter from CDPHE from April 2017 for clarification on exemptions: Immunization exemptions update from CDPHE (DOC). Non-medical exemptions must be submitted annually. Non-medical exemptions expire on June 30th of each year. Colorado Immunization Law
Colorado State Law requires students to provide an immunization record in order to be able to register for school. At registration, all students will be expected to have an immunization record.

Medications
All prescribed medication or over-the-counter medication needs to be checked into the health office with a signed doctor’s medication authorization form. Parents must sign district paperwork when checking in medications at school. The check-in process usually takes about 5-10 minutes.
Medication cannot be sent to school with your child or in your child’s backpack.
Please note that our GOAL is to keep all students in school, healthy and ready to learn. Headaches or menstrual cramps are frequent health complaints. Over-the-counter medications can be given with completed doctor’s forms to provide pain relief and prevent missed school. Contact the district nurse for more information. Every child must have their own supply of medication in the original container because the health office does not keep any medications on hand. Please check to make sure the medication is not expired.

Health Services Allergies
If your child has a severe Allergy, you will be given an Anaphylaxis Health Care Plan. Please sign and have it filled out and signed by your child’s doctor. Please check in allergy medicine with the Health Assistant or front office. A Self-Carry contract must be completed with the Allergy Health Care Plan if you, the nurse, and your doctor agree that your child can carry the Epinephrine device. If your child has an allergy and you DO NOT want to have medicine at school, please contact the school district nurse consultant.

School Medicaid Program
The Colorado School Health Services (SHS) program is run by the Department of Health Care Policy and Finance (HCPF) and the Colorado Department of Education. SHS is the Colorado state Medicaid program for schools. This is a federal-state matching program designed to help reimburse schools the cost of health services provided to low-income individuals;
